Admitting and Registration

People arriving at SRMC for both inpatient and outpatient services will register with the Admitting/Outpatient Registration Department, where friendly personnel will provide them with the information needed during their stay.


If you are being admitted to the hospital, use the Emergency/Admitting entrance on the west side of the hospital.
If you are scheduled for surgery, the pre-surgical nurse will call you to inform you what time and where to report at the hospital.

What to Bring:
Your insurance card and social security number will be needed to generate a medical record.
Make a list of all of your current medications, the dosages and prescribing physician(s). The list should include all prescription medications, herbal supplements, vitamins and over-the-counter products.
If you have a living will or advance directives, bring these documents with you.
Please bring only a few personal items, such as dentures, eyeglasses, a robe, slippers and toiletries. You may want to bring a small amount of money with you to purchase articles from the hospital's gift shop, located on the lobby atrium's second floor.
The hospital cannot be responsible for the loss or breakage of belongings that you bring with you to the hospital.

Outpatient Registration

If you are in need of an outpatient test or procedure, you will enter the hospital through the main lobby at the east end of the hospital. You will check in at the Outpatient Registration Department when you arrive at the hospital. The staff in Registration will verify or obtain billing and demographic information in order to generate your chart.

Please remember to bring your physician order for testing, if your physician's office did not fax it on your behalf. Bring your insurance card as well.

Scheduling Your Appointment

If your physician's office faxes orders for tests or procedures, but does not schedule this testing on your behalf, please call Central Scheduling at 330-332-7300. Their hours of operation are 8 a.m. to 5:30 p.m., Monday-Friday.

You will be asked to provide the patient's name, date of birth and dates available for testing. A Salem Regional Medical Center associate will work with you to schedule an appointment date and time that best meets your needs. By scheduling your tests, the paperwork needed for the day of testing will be completed before your arrival.

Walk-in Registration

If you are coming in from a physician's office, you may choose to bring your physician's order to the Outpatient Registration Department, temporarily located across from the Outpatient Laboratory on the hospital's ground floor. Please bring your current insurance card and be prepared to supply other information including your social security number, address, phone number and identification.
